Broken: Rebate instructions
The rebate depends on place where you purchased the product, yet they tell you to call the same exact number for both. What's the point?
I've always received rebates from everywhere I've applied for them. CostCo in particular is the best in rebate refunds. The trick is to make copies of what you send and when you sent it, and be sure to follow up if the rebate doesn't arrive in the specified time. Oh, and don't throw away junk mail without opening it first. I've almost thrown away a few rebates because I thought they were just junk mail.

Ha ha. You seem to be under the impression that businesses want you to cash in your rebate. The point is that rebates are a suckers game. They want you to take the rebate into account when you are deciding on a purchase, but they don't want you to actually claim your money. This is the same passive-aggressive corporate lip service as phone trees. If they thought my call was truly important they'd have a real human picking up.
